Open vs. Closed Rhinoplasty

There are two methods of performing Rhinoplasty: Open and closed. These terms refer to the way in which the underlying cartilage, bone, and soft tissue are exposed to allow for the procedure to be conducted.

In open Rhinoplasty, the framework of the nose is fully exposed.

Advantage

There may be more swelling that takes longer to resolve after surgery

Open Rhinoplasty is Preferred with

Cases of revision(redo) Rhinoplasty, significant asymmetry, unusual anatomy, or when there is a need for very precise modifications.

The closed technique is the more traditional method of Rhinoplasty that has been popular for decades. It involves making fewer incisions in the nose, and performing the alterations to the framework without seeing the structures directly.

Dry and Wet Paint

When you come in for a Rhinoplasty consultation, the following will occur:

  1. Dr. Ahmad will review your medical history, including any prior nasal surgery

  2. Dr. Ahmad will ask you about and listen to the specific concerns your want to address

  3. Dr. Ahmad will examine and evaluate your nose

  4. Dr. Ahmad will explain the various surgical maneuvers to the nose that will help achieve your goals

  5. Dr. Ahmad will present any other surgical maneuvers that he feels may enhance your rhinoplasty outcome and incorporate them based upon your input

  6. Dr. Ahmad will give his recommendation about which technique would be best for your goals

  7. Dr. Ahmad will go over the surgery process, recovery process, and the potential risks and complications of surgery The staff will provide you with a quote based on his recommendations and information regarding the logistics to having your surgery

Details of the Rhinoplasty Procedure

Dr. Ahmad performs rhinoplasty using one of two types of anesthesia: general anesthesia or IV sedation (also known as twilight anesthesia). With general anesthesia, patients are completely asleep and unaware throughout the procedure. Under twilight anesthesia, patients may experience brief moments of awareness but remain relaxed and feel minimal, if any, discomfort. The surgery is carried out at a private surgical center conveniently located near Dr. Ahmad’s plastic surgery office in Dubai.

For an open rhinoplasty procedure, Dr. Ahmad begins by making small, precise incisions on the nose to access the underlying cartilage and bone. From there, specific surgical adjustments are made based on the patient’s unique needs.

These may include:
  • Hump Removal: Cartilage and bone are carefully shaved to smooth out the nasal profile.

  • Tip Refinement: The tip's shape and definition are enhanced by removing, suturing, or adding cartilage.

  • Projection Adjustments: The nose's projection is increased or decreased to achieve balance.

  • Width Reduction: The nasal bones are repositioned for a slimmer, more refined appearance.

  • Septum Straightening: The septum is corrected through techniques like removal, scoring, or reinforcing cartilage to improve structure and breathing.

Once the desired results are achieved, Dr. Ahmad meticulously closes the incisions and applies a protective nasal splint to support healing. The patient is then gently awakened from anesthesia and provided with aftercare instructions to ensure a smooth recovery.

Recovery after Rhinoplasty

Recovery From Rhinoplasty Immediately After Surgery

Immediately after your rhinoplasty, you will be closely monitored by our nursing staff in the post-anesthesia recovery area. Once the effects of anesthesia have worn off, you will be discharged with a family member or friend to escort you home.

Recovery From Rhinoplasty at Home

At home, it’s important to rest and protect your nose from any accidental injury. Non-strenuous activities, such as watching TV or reading, are fine. Expect your nose to feel stuffy, with most patients needing to breathe through their mouth during the first week. Pain is typically mild to moderate and can be managed with prescribed medication. Swelling in the face and around the nose is normal, as are bruises around the eyes, which usually resolve within 1–2 weeks. Most patients can return to non-strenuous work within 7–10 days.

You may experience moderate bruising and swelling in the treated area, as well as moderate to severe discomfort lasting 7-10 days. Dr. Ahmad will provide a prescription for pain medication, which should be taken as needed. To ensure a smooth recovery, all prescriptions will be provided before surgery, and we encourage patients to have them filled in advance.

Follow-Up Visits After a Rhinoplasty

Dr. Ahmad will see you one week after surgery to assess your progress, remove stitches, and adjust your dressing. Light exercise, such as walking, is typically permitted two weeks after surgery, but strenuous activities and contact sports (e.g., basketball, hockey) should be avoided for 4–6 weeks to protect your healing nose. Follow-up visits with Dr. Ahmad are scheduled at 6 weeks, 6 months, and one year post-surgery to monitor your recovery and take postoperative photos.

Swelling is a normal part of recovery, and your nose may appear larger initially. Most of the swelling resolves within 6–8 weeks, but full healing, especially at the nasal tip, can take 12 months or more. Over time, you’ll notice the nose becoming slimmer and more refined, with the final results gradually emerging.

As with any surgical procedure, there are risks associated with rhinoplasty. While the risk profile is generally low, Dr. Ahmad will thoroughly discuss potential complications during your consultation to ensure you are fully informed and confident moving forward.

Asian and Ethnic Rhinoplasty

When Rhinoplasty is performed on a patient who is non-Caucasian, it is termed an ethnic Rhinoplasty. (Note that the term ‘ Caucasian,’ as it is applied to this discussion, refers to patients of European ancestry). Ethnic Rhinoplasty differs from non - ethnic (i.e. Caucasian) Rhinoplasty, as it requires consideration of two important aspects: cultural factors and anatomic factors.

Cultural Factors

Cultural concepts of beauty vary significantly and must be carefully considered when performing ethnic rhinoplasty. For instance, in Asian rhinoplasty, patients often request a higher nasal bridge, but the desired height is generally lower than that of a typical Caucasian nose. This highlights the importance of preserving the patient’s ethnic identity while addressing their aesthetic goals. Equally critical, however, is understanding whether an individual wishes to embrace or deviate from these cultural norms. Open and clear communication with each patient is essential to achieving successful outcomes in ethnic rhinoplasty. Additionally, the surgeon must have a deep understanding of cultural beauty standards across various ethnicities to tailor their approach appropriately.

Anatomic Factors in Ethnic Rhinoplasty

Ethnic rhinoplasty differs from Caucasian rhinoplasty due to distinct anatomical characteristics. For example:

African American Rhinoplasty

Patients often present with thicker skin, a wider alar base, and less cartilage support.

Asian Rhinoplasty

Common features include a low nasal bridge, minimal tip projection, a rounded or bulbous tip, and wide, flared nostrils.

Nasal Bone Structure

The nasal bones in ethnic patients are often different in shape and size compared to those in Caucasian patients, influencing surgical techniques.

These anatomical distinctions require surgeons to adapt their approach to suit each patient’s unique facial structure. Ethnic rhinoplasty demands expertise and a nuanced understanding of these variations to ensure that corrections align with the patient’s goals while respecting their natural features. This individualized approach is essential to delivering results that are both harmonious and culturally sensitive.

Revision Rhinoplasty

Sometimes, individuals who have undergone a rhinoplasty are disappointed with the results, finding that their initial procedure did not meet their cosmetic or functional expectations. Timing for Revision Rhinoplasty

Dr. Ahmad generally advises waiting at least 10 to 12 months after your initial rhinoplasty before considering revision surgery. This is because the nose continues to change during the first year—and sometimes even longer—due to gradual reduction of swelling (edema) and the softening of internal scar tissue.
